As introduced: On or before the 5th of each month, the district court clerk for each county shall provide a statement of each annulment or dissolution performed for the preceding month. The information requested by the clerk, and then given to the department, shall include: information regarding the legal custody, physical custody and parenting time determinations for each child (if any) who are affected by the action. This information shall come from the plaintiff or his or her legal representative and presented to the clerk of the court.
