I hereby agree and consent to the following: Voices for Children in Nebraska to use the name, image, and likeness of the minor Child(ren) in any photograph, video, or other digital media taken or to be taken in connection with the Child’s participation in Voices for Children’s programs, activities, and events (“Photos”) for all of Voices for Children’s publications, including print or web-based publications and social media. I specifically acknowledge and agree to the following:
1. I am a parent or designated guardian of Child(ren) and I have the legal authority to grant permissions described in this Photo & Media Release.
2. I grant permission for Voices for Children and its authorized representatives to photograph, videotape, record, copy, edit, enhance, crop, and/or otherwise alter any Photos of Child(ren) as related to the Child(ren)’s participation in Voices for Children’s programs, activities, and events.
3. I understand and acknowledge that Photos may be used in Voices for Children’s publications including, but not limited to: internal records, files, and program documentation; website and social media accounts; printed and digital marketing, brochures, or donor reports; or press releases and news coverage.
4. I waive any rights for approval or inspection of any Photos.
5. I understand and agree that all Photos are the exclusive property of Voices for Children and will not be returned to me.
6. I acknowledge that Child(ren) and I are not entitled to any compensation or royalties with respect to the use of the Photos.
7. I agree to release, discharge, and hold harmless Voices for Children and its affiliates, successors and assigns, officers, board members, employees, volunteers, representatives, partners, agents and anyone claiming through them, in their individual and/or corporate capacities, (“Releasees”) from any and all claims, liabilities, obligations, promises, agreements, disputes, demands, damages, causes of action of any nature or kind, which I or the Child(ren), and anyone claiming on behalf of me, may have or claim to have against Releasees in connection with this Release, other than claims arising from the Releasees’ gross negligence or willful misconduct.
This Release remains in effect unless I submit a written revocation to Voices for Children at voices@voicesforchildren.com. I understand that any such revocation will not affect the possession or use of Photos that occurred prior to Voices for Children’s receipt of written revocation.
I confirm I have carefully read and fully understand all the provisions of this Media Release Form and am freely, knowingly and voluntarily signing.
